Instructions for Providing Testing Accommodations

1. Facultyhave the primary responsibility for arranging proctoring and/or reduced-distraction environment for students with approved testing accommodations.

2. It is the student’s responsibility to request these accommodations from the faculty in advance of each test. If the student does not give sufficient notice for an test, accommodations may not be made available for that test.

3. If the faculty is not able to arrange the accommodations, the student needs to contact ODS as soon as possible prior to the test to reserve a room in Exeter 101 and to obtain a Test Proctoring Form to be completed by the faculty.

4. Distribution of the Test Proctoring Form:

White copy: faculty must send this copy with the test. The test will not be administered by ODS unless there is a completed and signed Test Proctoring Form.

Yellow copy: Student must return this copy to ODS prior to the day of the test.

Pink copy: Student’s copy for reference

5. Once the exam has been completed, the student and ODS will follow the Method of Return on the form.

IMPORTANT: 

If the student needs to use a screen reader for taking tests, an electronic copy (MS Word file, etc.) of the exam must be received in the Office of Disability Services at least 3 days before the scheduled exam.

Please deliver the electronic copy to ODS, Exeter Hall, Suite 101 or e-mail it to both

If the student feels that the prescribed accommodations are not being provided, he/she has the right to file a grievance with the institution's ADA/504 Compliance Officer.
